Intimation on choosing OSA for Family Floater Group Mediclaim

Here is letter sent by respective Pension disbursing authorities to all Pensioners too on their choosing OSA for Family Floater Group Mediclaim giving details . 

SZ in particular has regularised many operations impinging on Pensioners ,like Hly DR change letter with details,Pension Pay slip 1 or 2days before every month end, then we get from earmarked Bank the actual credit .all going fairly smoothly SZ TPA was the 1st & earliest to issue long  before Laminated Card for cashless service hospitalization Now only the xerox copy issued sent to all SZ Pensioners in lieu of card be shown to hospital who accepts this as evidence . 

2) Though in ZO,DOI AOs were on leave yet , AO PC SZO responded to my request & so I send the customary letter for OSA & Premium payable in usual installments from Pension sent by SZ
