How to Avail Cashless Service

Cashless hospitalization can be availed only at insurer network hospitals. TPA processes your request and guarantees the Hospital to pay the approved amount after discharge and submission of all required documents and information, subject to terms and conditions of the policy and approval of the Insurance company.

Steps to intimate claim online

Reimbursement Claim submission:

You can submit your reimbursement claim documents at your respective LIC branch office/Division office. Please find below link to download claim form. Need to submit below documents along with duly filled claim form at LIC.

DOCUMENTS REQUIRED FOR REIMBURSEMENT CLAIM SUBMISSION

  1. Duly filled original claim form with exact total claim amount
  2. Patient government id proof and LIC employee id card (photo copy)
  3. Original Discharge card, Final bill with breakup & All Cash Receipts with proper Numbering
  4. Original pharmacy, lab and consultation bills and prescriptions
  5. Original films, implant invoice and implant stickers/IOL stickers (depends on treatment)
  6. MLC (Medico Legal Certificate) copy and original alcohol influence certificate in case of accidental claims
  7. Hospital registration certificate copy (depends on Hospital type & Location)
  8. Original Physiotherapy chart (depends on treatment)
  9. Original Prenatal USG report in case of Maternity claim

Note: All Bills, Prescriptions, reports need to be signed & stamped by hospital/doctor/pharmacy/lab.
