CASHLESS PREAUTHORIZATION PROCESS
1. Please check if the hospital is in Network hospitals list using the link provided above.
2. Send the Pre-auth request through hospital at least 72 hrs before admission, on preauth @ghpltpa.com mail id with
Good Health id card, Patient govt id proof & pre-auth form (available with hospital).
3. In case of emergency admission please share Good Health id card, Patient Government ID proof with hospital
insurance desk, immediately during admission itself to send Pre-auth request.
MEMBER CLAIM SUBMISSION PROCESS AND CHECKLIST
1. Duly filled claim form with exact total claim amount, contact number and mail id
2. Patient govt id proof and employee ID card or health ID card
3. Original Discharge card, Final bill with breakup, package breakup (if any) & All Cash Receipts with proper
Numbering against the amount paid.
4. Original pharmacy, lab and consultation bills and prescriptions, films, implant invoice and implant stickers/10L
stickers (depends on treatment)
5. MLC (Medico Legal Certificate) copy, original alcohol influence certificate and complete accidental history (how
when and where it was happened) in case of accidental claims
6. Hospital registration certificate copy (depends on Hospital type & Location)
7. Original Physiotherapy chart (depends on treatment) and
8. Original Prenatal USG report in case of Maternity claim
** Note: All Bills, Prescriptions, reports to be signed & stamped by hospital / Treating doctor/ pharmacy / lab
(pathologist).
**Note: All member claims should be submitted at LIC DO only.
