In the illustrative chart sent by Executive Director(Personnel) along with his letter dated 11/8/2003 addressed to Mr G C Chaturvedi,Joint Secretary(Insurance & Banking),Ministry of Finance, Government of India,the following was the position of pre-August 1997 retirees:

The position was as follows as per the letter dated 20/2/2009 sent by Executive Director(Personnel) to Mr Sanjeev Sinha ,Under Secretary to Government of India,department of Financial Services,,Ministry of Finance ,New Delhi:

The number of pre-August 1997 retirees/ pensioners identified by LIC for payment of 40% interim relief as per the order of Supreme Court judgment in its judgment dated 31/3/2016 was 17963 as per the Revised Compliance Affidavit of LIC filed at the Delhi High Court on 22/7/2016.The number of pensioners who were actually paid 40 % interim relief was 15922.
There may be much more than 4000 pensioners to be accounted for in regard to payment of 40% IR if the correct number of retirees as at 1/8/1997 is taken into account.
I am shortly sending an application under RTI Act to LIC CO seeking the details.I shall share the reply when received.
The very fact that not a single family pensioner got 40% IR as family pension (leaving aside those who got single digit amounts) goes to show default by LIC not only in terms of numbers but also in terms of amount not in conformity with the Supreme Court Order dated 31/3/2016.
