MEDICLAIM FORM PART A TO BE FILLED BY THE INSURED

Respected Sir,

    My wife was hospitalized and after being discharged I submitted the claim on 6.10.17 utilizing the usual claim form. Now the TPA of our area, viz, M/s RAKSHA TPA asked me to provide them Claim Form Part A as per IRDA guidelines, which is available in their website www. rakshatpa.com.

    The OS Deptt of our Div. Office has no knowledge of any such new claim form. Even there is no information from our CO (who is the real insured  and purchaser of Master Policy every year) to our Divisional Office in this matter.

    I have down loaded the Form A and attach it for your perusal. You will find that many of the information called for therein shall not be available with the claimants.

     Sir, do you have any idea of such a change and if so kind enlighten me in at your earliest. It is possible that some of the readers of your esteemed blog may have some knowledge in this respect.
