MEDICLAIM FORM PART A TO BE FILLED IN BY THE INSURED

Dear Shri Verma,   The insurance companies have been using their own claim form for the purpose of reimbursement of hospital expenses.   NOW the IRDAI has standardized the Mediclaim form as per their new Health Insurance Guidelines, 2016 issued in July 2016.  The policy number is mentioned on the ecard.. The insured is LIC of India central office Mumbai.   The Policy No. For NCZ is  120700341170400000004  The date of inception of Mediclaim insurance can be left blank.  The OS Department may fill the same.   I feel there should be no difficulty in filling in the Form A.   With regards,.  R K SAHNI
